A mentoring relationship for a one-year commitment and both parties in the relationship have reported success.

Definition of short-term success:

Successful monthly activities and completion of the 3-month review.

Success monitored by:

Case Managers.

Evidence of success:

Positive mentor relationship evaluation reports.
