Adult Rehabilitation Ministry
Golden Gate Adult Rehabilitation Ministry (ARM) has dedicated the last 27 years to transforming chemically dependent individuals through loving, accountable relationships and a Christ-Centered twelve step process in a residential treatment program. Funding will support clients who cannot afford the cost of entering the program as well as further development of the program. ARM helped 45 men in 2017, with an average stay of 164 days.

ALS Association of Texas
The ALS Association of Texas works to discover treatments and a cure for ALS, and to serve, advocate for, and empower people with ALS to live their lives to the fullest. Funds are requested to establish ALS multidisciplinary clinics in North Texas, maintain, and enhance equipment loan closet inventory, and sustain the Continuum of Care program to patients.

BIND: Brain Injury Network of Dallas
BIND: Brain Injury Network of Dallas BIND operates the first and only post-rehabilitation program for brain injury survivors in the state of Texas. Their mission is to provide a bridge of support for adult brain injury survivors so they can reconnect to work, life and the community. Funding is requested to help expand their Work Readiness program, open a second location, and expand program staffing in areas of direct program support, volunteer service coordination, and development. These efforts will help to reduce the program waiting list.

Cancer Care Services
Cancer Care Services serves thousands of individuals affected by cancer annually, in Tarrant, Parker, and Hood counties, by supporting cancer patients, caring for caregivers and empowering survivors. Funds are requested for: 1) COPE programs: Health Insurance Assistance, Prescription Co-Pay Assistance, Nutritional Supplements, Dietitian Services, Medical Supplies, Case Management/Navigation; 2)THRIVE programs: Nutritional Education, Wellness Workshops; and 3) CONNECT programs: Client Activities, Family-centered programs.

Center for BrainHealth at UT Dallas
In 2017, the Center for BrainHealth’s Brain Performance Institute developed an evidence-based, culturally competent mindfulness program for law enforcement. Mindfulness improves resilience and emotional self-regulation, strengthens attention, and promotes well-being. Self-regulation in highly-charged situations is essential for those who work in high-tempo operations professions such as law enforcement. Additional funding will help train officers.

Children and Community Health Center
The Children and Community Health Center (dba Community Health Clinic) provides comprehensive, coordinated care with a focus on health, education, preventive care, and lifestyle improvements to effect change in the lives of low-income, uninsured residents of northern Collin County. They provide services at no cost to the patient and have collaborative partnerships for labs and imaging services at no charge. Funding is requested for medical supplies, medications and patient cost of $75 per visit.

Children's Medical Center Foundation on behalf of Children's Health
Children’s Health provides pediatric care for more than 300,000 children annually. They offer quality clinical care, psycho-social support programs, and pioneering pediatric research. Current funding priorities include the renovation of the Emergency Department on their Dallas campus, clinical research, and support for the non-revenue-generating patient and family support services.

Community Hospice of Texas
Community Hospice of Texas (CHT) is the most extensive non-profit hospice provider in Texas. Their mission is to provide compassionate end-of-life care and services. CHT offers a care model for the physical, emotional and spiritual well-being of the patient and their family and friends regardless of their ability to pay. Funds are requested to help provide hospice care for the uninsured who are facing a life-limiting illness.

Dallas Life, Inc. dba Thrive Women's Clinic
Thrive Women’s Center extends Christ-like love compassionately and responsively that values the family and empowers women and their partners to choose life. Funding is requested for the Healthy Behaviors Educational Medical Program, which provides non-judgmental care to women experiencing an unplanned pregnancy. Thrive Women's Center's goal is to help women find hope, answers, and information immediately. Last year their nurses conducted 1,806 pregnancy tests and performed 1,572 sonograms.

Dental Health Programs, Inc. dba Community Dental Care
Community Dental Care works to improve the oral health of all by providing low cost or free dental services. Existing programs allow them to serve specific populations at little-to-no out of pocket expense. A focus on routine, preventive care helps mitigate the impact of poor oral health on the region as a whole by reducing emergency room visits and days missed at school or work caused by dental pain. Funds are requested to help the agency serve those for whom they cannot provide grant-funded care.

Denton County MHMR Center
Denton County MHMR Center provides critical public safety services for crises prevention, intervention, and rehabilitation for individuals with serious and persistent mental illness and developmental disabilities. Last year, the organization served 5,882 low-income individuals. Funds are requested for the LOSS Team, which provides services to individuals and families impacted by suicide. Also, funds are requested for Holiday Helping Hands, which provides providing holiday assistance to clients and their children.

DKMS
DKMS dedicates itself to the fight against blood cancer and blood disorders by creating awareness, recruiting bone marrow donors, raising funds to match donor registration costs, supporting the improvement of therapies through research, and supporting patients. Funds are requested to support the recruitment of young males and ethnically diverse individuals by funding registration costs, development of educational materials, and identifying areas of Dallas to host drives that target potential lifesaving donors.

Founded in 1935, Drug Prevention Resources (DPR) helps parents and kids navigate the turbulent teenage years through drug-prevention education and community coalitions. Global Breast Care Initatiative
Global Breast Care Initiative works with local community leaders to overcome proven barriers to healthcare including distance and lack of transportation. The "Texas Scanning Project" uses a pain-free, radiation-free, handheld, mobile device to provide free community-based breast health screening within statistically identified low income or low resource areas. Funding priorities include direct support for mobile screening expenses, breast health navigation, and a text-based breast cancer resource program.

Grant Halliburton Foundation
Grant Halliburton Foundation’s mission is to strengthen the network of mental health resources for children, teens, and young adults; promote better mental health, and help prevent suicide. Funds are requested for educational presentations delivered in the classroom to teach students about mental illness, and how to recognize and respond to a crisis. Additionally, funds are requested to launch a no-cost psychological health navigation line to help individuals find appropriate care for mental illness.

Healing Hands Ministries
Healing Hands Ministries' primary-care, full-service clinic assists 2,000 low-income children and families each year. Funding will support medical, dental, and behavioral health services for children. Their goals are to improve children’s health by increasing child visits by 5%; assist underserved children in obtaining tools to improve health status and quality of life by offering asthma education, diabetes education, and obesity prevention; and reduce non-emergent Emergency Room visits by 3%.

Health Services of North Texas, Inc.
Health Services of North Texas (HSNT) provides quality primary medical care and support services to create a continuum of care for underserved individuals, prevent unnecessary emergency department visits, and address social determinants of health. In 2017, HSNT provided medical care to 14,148 patients. Funds are requested for a portion of the cost of medical visits for uninsured patients using the reduced-rate, sliding-fee scale, as well as patients with chronic disease and mental health issues.

Homeward Bound
Homeward Bound treats about 6,000 impoverished people each year struggling with substance abuse and mental illness. Their services range from detoxification to outpatient services. Recovery coaches help throughout the recovery journey and the program's results are consistently above the national average. Funds are requested for: 1) a car/van to drive clients to appointments and classes; 2) a picnic area/playground to engage children of clients/parents during family education, CPS and other visitations; and 3) client laundry facilities.

Journey of Hope Grief Support Center
For 20 years, Journey of Hope Grief Support Center has provided support at no cost for children and their families as they learn to mourn the death of their loved one in a safe, caring, and nurturing environment. By intervening at a critical point in the grief process, they are both intervention and prevention for the risk factors associated with grief. With nearly 400,000 children in North Texas who are grieving the loss of a parent or brother or sister, families drive to them from ten counties to find help. Funds are requested for general support.

Legacy Counseling Center
Legacy helps people living with HIV reclaim their lives and thrive by providing mental health care, substance abuse treatment, and housing assistance. Legacy cares for and teaches healthy habits to over 1,100 positive people annually, resulting in reduced new infections and a healthier, happier community. Funds are requested for 1) operating support, 2) counseling, including a 24-hour hotline, and 3) scholarships for positive women to attend our 3-day leadership conference.

Leukemia Texas
Leukemia Texas improves the lives of Texans with leukemia in two distinct ways: 1) through the Patient Aid Program that provides crucial financial support, up to $1,250 per leukemia patient annually, to offset the expensive cost of life-saving treatments and provide for basic needs (medications, transportation, housing costs, food) and 2) through funding of medical research towards innovative therapies and an eventual cure. Funds are requested for the Patient Aid Program or Research Program.

Life Message hopes to provide low-cost healthcare, including medical, vision, dental and counseling services, as well as hot meals and after-school programs. Life Message aims to triple the number of clients served over the next year, offer more types of professional services and to help thousands more families across the Northeast Texas Region regain their health and food stability. Proven outcomes, professional standards, and a family atmosphere have led to high demand for ManeGait programs. Funding is needed to serve ManeGait’s growing waitlist, expand their innovative “GaitWay to the Brain” program, and support their “ManeGait to Freedom” program dedicated to helping veterans and first responders.

March of Dimes
March of Dimes fights for the health of all moms and babies. Unfortunately, more than 10% of babies born in Texas are premature. March of Dimes supports moms throughout their pregnancy, even when everything doesn’t go according to plan. They also advocate for policies that prioritize maternal health and support radical improvements to prenatal care. Funding will be used to provide prenatal education classes, outreach to hospitals, doctors, and nurses in the DFW area, and consumer awareness on the importance of prenatal care.

Mental Health America of Greater Dallas
Mental Health America of Greater Dallas' mission is to lead the community in improving mental health through advocacy and education and become a safe place that understands psychological health concerns and supports people with mental illness in reaching their potential. Funds are requested for its WHO (We Help Ourselves) program, an anti-victimization curriculum delivered in schools. Funds are also requested to update and expand the proven program's content to new grade levels. Over 107,000 youth were served by the program last year.

Methodist Richardson Medical Center Foundation supports the growth of the Richardson hospital. Their 326 physicians offer expertise in 35 specialties. Last year Methodist Richardson served patients from 45 counties in Texas and five other states. Funds are requested to purchase equipment for the Center for Surgeries and Digestive Diseases. Additionally, funds are requested for the Asian Breast Health Outreach program, targeting Asian women who are susceptible to breast cancer.

Mothers Against Drunk Driving - North Texas Affiliate
The North Texas Affiliate of Mothers Against Drunk Driving is the premiere nonprofit working to eliminate drunk driving, help fight drugged driving, support the victims of these senseless crimes and prevent underage drinking. Through education, they help avoid needless tragedies and provide supportive services to victims at no charge in 31 North Texas counties. Funds are requested for new/on-going services to victims/survivors, outreach to law enforcement and hosting Power of You(th) program.

NAMI Dallas works to educate, advocate and support communities to relieve the effects of severe mental illnesses, now known as biological brain diseases, on individuals, family members, and society. NAMI Dallas supports communities by providing education classes for peers in recovery, courses taught by families for families, and youth programs for students and educators to recognize signs of mental illness. Open Arms Health Clinic
Open Arms Health Clinic provides high quality, compassionate healthcare to uninsured and underinsured individuals and families in North Texas. Located in Arlington, their all-volunteer medical and support staff provided over 3,000 free medical and counseling appointments to almost 550 unique patients in 2017. Funding will support the cost of medical supplies and other operating expenses. As an all-volunteer organization, 100% of donations go to patient care.

Parkland Foundation secures financial resources that advance the clinical, educational and research quests of Parkland Hospital. Funds are requested to provide free screening mammograms, diagnostic services, and transportation to women whose income is at or below 200% of the Federal Poverty Guidelines and who are uninsured. The lives of medically underserved Dallas residents without access to healthcare will be saved by screening and treating them earlier for breast cancer.

PediPlace
PediPlace provides pediatric healthcare for uninsured or underinsured children, who would go without care or would unnecessarily use the E.R. With clinics in Lewisville and Dallas, PediPlace served 6,581 unduplicated children in 24,634 office visits in 2017. Funds are requested: 1) to provide free and low-cost services to families served; 2) for the Healthy Babies program which offers free access to all 6 recommended pediatric visits; and 3) to the Reach Out & Read program to purchase new books.

Planned Parenthood of Greater Texas (PPGT) creates healthier communities by providing comprehensive reproductive and related healthcare services, delivering science-based education programs, and serving as a strong advocate for reproductive justice in Texas. In 2017, PPGT provided healthcare services and health education to more than 85,000 Texans. Funding is requested to support core programming for healthcare services, health education, and advocacy efforts for reproductive justice.

QuestCare Foundation
In 2013, Questcare Foundation launched the Questcare Clinic in partnership with Watermark. The clinic provides urgent care services and in 2017 had 9,750 patient visits, 86% of which were for patients with no insurance. Conservatively, the citizens of Dallas who would have nowhere else to turn but the city's ERs saved $10.2 million because of the clinic's service. Funds are requested to help launch a second clinic serving the residents of Collin County in partnership with Watermark Plano.

Relationships First is powered by Safe Conversations, a 3-step process (mirroring, validating and empathizing) which gives individuals, couples, and families the tools to form relationships that are safe, valued, and connected. Funds are requested to: 1) provide workshops and training to ecosystems they serve, which include schools (teachers, students, parents), first responders, families, veterans and those in transitional housing; 2) translate materials into Spanish; and 3) digitize "Train the Leader" program.

Simply Grace
Simply Grace's mission is to provide chemically dependent women a structured, safe, homelike environment where self-realization and honesty are encouraged and reinforced. Funding is requested for Simply Grace's program for women who are battling substance addiction in the Dallas/Plano area. On average, eight residents can reside at each of Simply Grace's two Dallas and two Plano houses at the same time, and Simply Grace will serve a total of about 160 residents annually in the four homes.

Stronger than Espresso empowers survivors of domestic violence and sexual abuse with practical tools to reclaim what abuse took away. Stronger is a catalyst for life-changing emotional healing and restoration and spiritual growth. Every Stronger session is written for survivors by survivors. The TeamConnor Childhood Cancer Foundation's mission is to raise funds for national childhood cancer research projects, to build awareness of childhood cancer, and to provide patient programming. Texas Health Resources is a faith-based nonprofit health system with 29 hospitals and more than 350 access points across North Texas. Still, there is more needed to address the health of our communities. In 2018 they launched Texas Health Community Impact, a bold new vision to invest in community health through collaboration. Texas Scottish Rite Hospital for Children is a leading pediatric care center specializing in the treatment of orthopedic conditions and related neurological disorders, and learning disorders like dyslexia. All services, from outpatient clinic visits and inpatient surgeries to on-site therapies and rehabilitative programs, are provided to children regardless of any family's ability to pay. Funds are requested to support patient care efforts at the hospital's central and North Campus locations.

The Cooper Institute (CI) dedicates focus to promoting life-long health and wellness through research and education. CI houses the Cooper Center Longitudinal Study that evaluates the health benefits of physical activity and lifestyle. Funds are requested for: 1) their researchers to focus on healthy aging; 2) to establish a renowned scientific advisory board that uses the CCLS and take research from the regional level to national; and 3) to secure the latest technology in managing large research data warehouses.

The Ryan Anthony Foundation /Cancer Blows
The Ryan Anthony Foundation / CancerBlows TRAF raises money for blood cancer research through musical events featuring the talents of legendary brass musicians like Doc Severinsen. The study supports new therapies such as CAR-T which offers patients a possible cure when their disease no longer responds to existing treatments. Funding is needed for production costs for a concert featuring the next generation of players, to support the cost of a Director, and to update the website.

The Well Community
The Well Community provides hope and healing to low-income adults living with mental illness through healthy relationships, essential services, and meaningful work. Funding is requested for the Our Daily Bread program that provides prepared meals to ensure clients receive proper nutrition, and the transportation program that offers door-to-door van service and bus passes to ensure clients have access to programs and services. Last year, they served 298 low-income adults with chronic mental illness.

University of North Texas Health Science Center (UNTHSC) Pediatric Mobile Clinic
The UNTHSC Pediatric Mobile Clinic provides free health care services on board a medical mobile unit to children 0-18 in Fort Worth who are uninsured or underinsured. Funds are requested to: 1) implement a teen clinic day by funding an OB/GYN healthcare provider once a month; 2) training for our clinical team on trauma-informed health care; and 3) patient care. Since 2014, the program has served over 5,500 children providing medical services, vaccines, and screenings.

VNA
Since 1934, the Visiting Nurse Association of Texas (VNA) has been providing the highest quality care in the home across North Texas, especially for those with no place to turn and no means to pay. VNA delivers Meals on Wheels in Dallas County and Hospice Care and Palliative Care with the new VNA Care Choices program in 13 North Texas counties. VNA ensures the homebound, elderly, and disabled in our community receive expert and compassionate care.
